Komparativna analiza nutritivnog statusa i kvaliteta života kod pacijenata sa oralnim karcinomom sa i bez suportivne terapije
Patients with oral cancer, in most cases, already have a certain degree of nutritional status disorder and body mass loss at the time of their first examination. Additional surgical and chemoradiation therapy lead to changes in nutritional status and reduced intake of macro and micronutrients. This all leads to reduced effectiveness of oncological therapy and longer hospitalization. Therefore, timely recognition of malnutrition and its prevention is an important part of the treatment protocol for patients with oral cancer. Given all the above, this type of problem deserves special clinical attention and an interdisciplinary approach to treatment.The aim of this research was to evaluate the results of nutritional status and quality of life in patients with oral cancer with or without supportive therapy, and to adopt a methodology for analyzing nutritional status and creating an individual plan of supportive therapy and dietary plan. Additionally, to compare the results of nutritional status and quality of life in patients with or without supportive therapy in relation to: body mass index (BMI), serum albumin, prognostic nutritional index (PNI), nutritional risk index (NRI), frequency of malnutrition and quality of life in both groups in terms of swallowing ability, speech, pain, taste, etc.The research was conducted in the Maxillofacial Surgery Service of the Clinic for Dental Medicine Niš. This study included 50 patients with oral cancer. In 25 respondents, nutritional supportive therapy was used as a supplement to the standard hygienic-dietary regime during surgical-oncological treatment, and 25 patients were on a standard hygienic-dietary regime without supportive therapy. Appropriate statistical tests were utilized for comparison of the tested parameter values between the groups.The results of this doctoral dissertation have great significance and applicability in terms of using therapeutic modalities in the analysis of nutritional status and quality of life, and the importance of applying supportive therapy in patients with oral cancer. Standardizing parameters for analyzing nutritional status and quality of life reduces the average time for their analysis. BMI, NRI, and PNI are the three most important parameters for analyzing nutritional status. The results of this study indicate that the application of supportive therapy significantly improves nutritional status, prevents the occurrence of malnutrition and subsequent complications. It also improves the quality of life in patients with oral cancer.
