Extracellular vesicles as biomarkers of disease activity in antineutrophil cytoplasmic antibody-associated vasculitis: doctoral dissertation

The purpose of this scientific investigation was to evaluate the degree ofinflammation and disease activity using activity markers on extracellularvesicles in patients with ANCA-positive vasculitis, and to compare findingswith healthy controls. The research was conducted in collaboration withKarolinska University Hospital, Solna, Sweden, within ERASMUS plusexchange program between University of Niš and Karolinska Institute. Data andbiological samples were obtained from the VASKA cohort, which includedpatients with active (defined as BVAS≥1, infection excluded) and inactiveANCA vasculitis, i.e. in remission (BVAS=0). Age- and sex-matched healthyindividuals served as control subjects. Concentration and phenotype ofextracellular vesicles in plasma were assessed by flow cytometry, in both activeand inactive disease, and compared to healthy controls. In addition, the procoagulantproperties of extracellular vesicles were examined through their effecton thrombin formation, using a modified method based on CalibratedAutomated Thrombogram (CAT) assay. The results demonstrated significantlyelevated levels of extracellular vesicles bearing PTX3, HMGB1, TWEAK andcomponents of complement system (C3a and C5a) in patients with active diseasecompared to those in remission and with healthy subjects. This studydemonstrates that circulating extracellular vesicles are significantly elevated inactive AAV and correlate with clinical disease activity (BVAS). These findingssupport their potential role as biomarkers for disease monitoring.
